Mobile Health Unit

 

Mobile Health is care that goes beyond four walls. Our Mobile Health Units are customized motor vehicles that travel to communities to provide access to various preventative services, including COVID testing and vaccinations.

 

Primary Care Mobile Health Clinics

Services for the below events include:

  • Primary Care Services
  • Minor Sick Visits
  • Medication Management and Education

 

Emlenton Mobile Health Services

  • Starting on January 7th , we will offer services including primary care services, minor sick visits, and medication management and education from 9:00 am to 3:00 pm every Tuesday at 516 Main St, Emlenton, PA 16373
  • Patients interested in scheduling for Emlenton mobile health services should call 724-704-4358

Empowering Health & Guiding Recovery Mobile Health Clinics:

The Empowering Health & Guiding Recovery program is a Mobile Health Assessment and Opioid Referral Program, led by our organization in collaboration with the Mercer County Behavioral Health Commission. It’s important to clarify that this program is not an Opioid Treatment Facility. Rather, it is a transformative initiative designed to provide crucial outreach and support to individuals in a nonjudgmental and secure environment.

Services for the below events include:

  • Opioid Use Disorder Treatment Referrals
  • Medical Needs Assessment and Treatment
  • Assessment and Links to Mental Health Services
  • Referrals to other services and assistant programs

We offer a sliding fee discount to all income-eligible underinsured or uninsured patients.

Telehealth video or phone visits allow for patients to safely connect to our healthcare services from the comfort of their own home.

Primary Health Network participates in most insurance plans including, but not limited to, Medicare, Medicaid, and CHIP.
