  • Get information about PHN’s locations, services and providers.

Locations

  • Sort locations by county, distance or alphabetically.
  • Tap on specific location cards to go to that location’s detail view.

Location Details

  • Get location-specific information including hours, phone numbers and affiliated providers.
  • Get directions by tapping on the location image caption.

Provider Details

  • Get provider-specific information including their bio and affiliated locations.
  • Tap on the affiliated location card to go to location detail view.

Patient Portal

  • Sign into your account to schedule appointments, renew prescriptions, pay bills and more.
