Sandy Lake– The Primary Health Network (PHN), a growing federally qualified health center, is set to offer family medicine services to the Sandy Lake community. The office of Dr. Martha Moore of 3205 South Main Street, will transition from Hometown Medical Care to The Primary Health Network on December 1, 2015. The transition will have a minimal effect on existing staff and patients, and it will allow for future growth of the practice. The Primary Health Network was founded over 30 years ago with one small site and one part‐time provider. Today, we’ve grown to become the largest community health center in Pennsylvania, with over 45 sites spanning across 14 counties in western Pennsylvania and eastern Ohio. PHN accepts most insurances, welcomes those under Medicare and Medicaid insurance plans, and offers a sliding fee pay scale to those who qualify.As our country’s future healthcare system will present new challenges, along with opportunities, Primary Health Network stands ready to meet the healthcare needs of all residents in the communities it serves. The Primary Health Network’s Sandy Lake Community Health Center will be open Monday, 8:00 AM – 8:00 PM and Tuesday through Friday, 8:00 AM-5:00 PM.
