As part of a grant through the Substance Abuse and Mental Health Services Administration, Primary Health Network (PHN) and Community Guidance Center (CGC) are partnering to offer integrated care at PHN’s Jacksonville Family Medicine Center. Integrated care is a model of healthcare that delivers primary care and behavioral health services in one location. This model of care has been shown to increase access to services, improve patient outcomes, and even reduce the stigma surrounding mental health.
Research shows that physical and mental health are closely linked; people with chronic physical conditions can be at higher risk of developing mental health conditions, and vice versa. Approximately two-thirds of adults with mental illness also have a chronic health condition such as high blood pressure, diabetes, or heart disease. With this in mind, integrated care allows primary care and behavioral health providers to easily communicate about the patient’s care and treatment plan. It is also convenient for the patient, allowing them to receive care in one place during a single visit.
Primary care services are provided by PHN physician Dr. Haney Wahba at Jacksonville Family Medicine Center; behavioral health services are provided by CGC licensed behavioral health providers and include intake, assessment, therapy, and psychiatric referrals. Both organizations have strong roots in Indiana County and are committed to improving the health and well-being of the community.
