Primary Health Network complies with applicable Federal civil rights laws and does not discriminate on the basis against individuals based on characteristics protected by applicable federal laws.

Primary Health Network:

  • Provides free aids and services to people with disabilities to communicate effectively with us, such as:
    • Qualified sign language interpreters
    • Written information in other formats (large print, audio, accessible electronic formats, other formats)
  • Provides free language services to people whose primary language is not English, such as:
    • Qualified interpreters
    • Information written in other languages

If you need these services, please inform the office staff who will arrange for these services to be available.

If you believe that Primary Health Network has not provided these services or has discriminated against you in any way, you may file a grievance with the Primary Health Network Compliance Department, located at 50 Pitt Street, Sharon, PA 16146. You may also submit your grievance by calling the Compliance Hotline at (877) 251-2954, sending a fax to (724) 704-4665, or by completing the online form at: https://my.compliancehotline.com/report/PrimaryHealthNetwork.

You can file a grievance in person or by mail, fax, or email. If you need help filing a grievance, please call PHN’s Customer Service Line at 1-866-276-7018 where a customer service representative will gladly help you.
